The short version: This notice describes how medical information about you may be used and disclosed, and how you can get access to it. Please review it carefully. In plain words: we use your health information to care for you, to get paid by your insurance, and to run the clinic — and for almost anything else, we ask your written permission first.

How we may use and share your health information

  • For treatment. We use your health information to care for you, and share it with other providers involved in your care — for example, when we refer you to a specialist or send a prescription to your pharmacy.
  • For payment. We share information with your health plan to bill for services — for example, sending a claim with the visit’s diagnosis codes to your insurer.
  • For clinic operations. We use information to run the practice safely — appointment reminders, quality checks, and staff training. Reminders never include diagnoses or test results.
  • When required by law. We disclose information when law requires it: public health reporting, abuse or neglect reports, court orders, and similar situations.
  • Everything else needs your written permission. Marketing, sale of information, and most sharing of psychotherapy notes never happen without your signed authorization — and you can revoke it at any time.

Your rights

  • See and get a copy of your record. Ask us in writing; we usually respond within 30 days. Electronic copies through the portal are free.
  • Ask us to correct your record. If you believe something is wrong or missing, you can request an amendment in writing.
  • Request confidential communications. Tell us how and where to reach you — for example, only on your cell phone.
  • Ask us to limit what we share. You can request restrictions. If you pay for a service fully out of pocket, you can require us not to tell your insurer.
  • Get a list of disclosures. You can ask for an accounting of the times we shared your information outside of treatment, payment, and operations.
  • Get a paper copy of this notice. Any time, at the front desk — even if you agreed to receive it electronically.

Our duties

We are required by law to keep your health information private and secure, to give you this notice, and to follow it. If a breach ever compromises your information, we will notify you promptly. When this notice changes, the new version will be posted in the office and on this page.

This website, cookies & analytics

This website never collects, stores, or asks for health information. Medical questions, records, results, and appointment details live in the patient portal, which is protected under HIPAA and covered by a Business Associate Agreement.

We use a small number of first-party cookies to understand overall site visits (for example, which pages are read most). We do not use advertising trackers or third-party analytics pixels anywhere on the site. The site is served over an encrypted connection (HTTPS).

Questions or complaints

If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will never retaliate against you for filing a complaint.
